Ten lives:
Press Start at the title screen to display the start and option selections. Press B, A, B, A(2), C, A(2), Start.

Level select:
Play through the game until reaching stage 2-4. Allow the introduction sequence for that stage to complete. After this point, you can hold A + C + Left and press Start at the title screen to display the level selection option.
